Sometimes the Results are Worth the Wait – M’s Story

How did this start?

M (name omitted by request) has been a loyal patient of Eggert Family Dentistry for nearly 20 years, since even before Dr. Elizabeth’s time.  In 2013, M underwent jaw surgery, also known as orthognathic (or-thog-NATH-ik) surgery, which corrects irregularities of the jaw bones and realigns the jaws and teeth to improve the way they work.  

In December 2015, during a recare visit, he mentioned that he had to force his muscles to bite correctly and wondered if that was normal after jaw surgery.  He also noted that his teeth were jagged and did not have a clean line.  He didn’t want to think about them all the time, like when eating or when looking at himself in photos.  

At that time, M decided to undergo our records process to determine how lengthening his front teeth would help improve his bite.  While M’s bite was improved after his orthodontics and jaw surgery, there had been enough prior damage to the teeth and the joints that M still had some bite imbalances after he healed from his surgery.

After working up M’s case, Dr. Elizabeth determined that with six upper veneers to lengthen his front teeth and some minor equilibration to even out high spots in his bite, M should feel better about both how his teeth fit together and with how they look.

What did M want?

M wanted to get rid of the gap he saw between his lateral and canine incisors and he wanted no chips on the front teeth.  He wanted a whiter and brighter smile as well without all the white lines on his natural teeth.   

What was involved? 

Dr. Elizabeth started the case by mocking up the new shape of the teeth on dental models.  M really liked the result.  We started the treatment by evening out M’s bite with equilibration.  Then, impressions of his new bite were taken and sent to the dental lab so a lab wax-up could be completed.  This gave us the actual blue print for his new teeth.  In the interim, M underwent the Zoom In-Office Whitening procedure to brighten his teeth to the desired color for his new veneers.  Soon after the whitening procedure, M came for his preparation appointment and then 2 weeks later, his veneers were carefully inserted. 

What does M think? 

In his final interview, M was asked how he felt about his course of treatment and his decision to complete the veneers.  He said, “I was concerned about it being painful or that there were going to be a lot of appointments to complete the treatment.  While there were a lot of appointments, the time went quickly and it was painless.”  He is very happy with the outcome and would recommend that anyone thinking of getting veneers to just get them done!  He said it was easy to have an open dialogue with Dr. Elizabeth and it was great to have a solid plan along the way.

M did say that he hasn’t had many comments on his veneers, but he’s glad about that because he wanted everything to look very natural.

Avoid Staining Foods

If your teeth are prone to discoloration, you can improve matters by avoiding acidic, sugary, and dark-colored foods. Major culprits include coffee, wine, berries, and chocolate. If you do choose to indulge, rinse your teeth as soon as possible afterwards and make sure to brush with regular intervals. Better still, consume these in moderation and load up on fresh fruits and veggies, and enjoy cheese and dairy for the beneficial calcium they offer as they are less likely to stain your teeth.

Stay Away From All Types of Tobacco

Tobacco stains teeth, including vaping and chewing tobacco. If taking care of your physical health doesn’t motivate you enough to quit, maybe vanity can! Tobacco is a prime cause of yellow, dingy teeth, so please quit. There are many effective cessation programs if you need help.

Preventative Care at Home

We will never stop singing this same old tune: Brush and floss, brush and floss. Your preventative care is the best way to keep your teeth healthy, and healthy teeth are attractive teeth. Brush and floss twice daily to combat gum disease, tooth decay, and plaque buildup. And please, do both! It’s impossible to brush the tight interproximal areas in between your teeth, so floss every time you brush. For anyone wondering whether your dentist can tell if you floss and brush routinely, the answer is YES. We can.

Get Regular Cleanings by a Professional

No matter how well you brush, you will get some tarter buildup, a by-product of plaque. When plaque builds up on teeth, it hardens and turns into tartar or calculus. It requires professional cleanings to scrape it off your enamel. Please get twice annual dental cleanings, and remember that plaque buildup doesn’t reflect on any brushing deficit – it happens to everyone.

Get Your Teeth Whitened

ICON: Sometimes teeth develop white or brown spots. This can happen for several reasons. If your teeth were exposed to too much fluoride as a child, you eat foods that stain teeth or smoke cigarettes or in some cases, if you suffer from Celiac Disease, these unsightly spots may occur. Fortunately, Dr. Elizabeth and Dr. Jeff offer a minimally invasive treatment that reduces the appearance of these spots. During an ICON treatment, Dr. Elizabeth and Dr. Jeff infuse teeth with a resin that lightens teeth without the need to reshape or remove enamel in any way.

Tooth replacement: Losing permanent teeth can happen from trauma to the jaw, from gum disease or from tooth decay. Regardless, it can be an awkward situation and one that most people want to remedy as quickly as possible. If you’re dealing with missing permanent teeth, you have some excellent options. These options include dental implants, bridges, partial dentures or full dentures.

Dental implants: With dental implants, a titanium post is fixed into the bone where the tooth was and the jaw bone grows back around the implant, holding it firmly in place. Then, an abutment and crown are fitted on top of the implant.

Bridges: Just like it sounds, bridges “bridge” a gap caused by missing teeth and use the neighboring teeth as anchors. These false teeth can stand in for one or more missing teeth. However, bridges can cause stress on neighboring teeth and food can get trapped underneath them, making cleaning more difficult.

Partial dentures: Partial dentures or “partials” replace one or more teeth. These false teeth attach to a plastic base and often have a metal framework. They are fairly comfortable and easy to remove for cleaning.

Full Dentures: Full dentures are full sets of upper and/or lower artificial teeth that are suctioned into place and removable for cleaning. While they can take some getting used to, they start to feel more normal over time. Full dentures will eventually become loose as bone mass degrades.

Tips for Whiter Teeth Between Your Recare Visits

It’s not disputed that whiter teeth lend themselves to a more attractive smile. Statistically, more than 66% of Americans recall attractive features about someone more easily than they recall unattractive features. Thus, a white smile and straight teeth leave a stronger impression than a dull smile and crooked teeth. In fact, Americans make the unconscious assumption that people with straighter, whiter teeth are more intelligent, even though there is no basis for this. These findings demonstrate the power of a brilliant smile to make a lasting impression. If you’re feeling self-conscious about your dull smile, there are quite a few avenues you can pursue to regain and maintain a whiter smile this year.

Tooth discoloration prevention

The biggest investment you can make in your dental health is in the way of prevention. There are several things you can do to prevent tooth discoloration:

Avoid foods and beverages that stain teeth: Many foods are intensely pigmented, have substantial tannins or are highly acidic and easily stain enamel. Avoid or limit the consumption of berries, red wine, tomato-based products, dark chocolate, coffee, tea, soft drinks, energy drinks and sugary treats. If you consume these, wait a half an hour to let your saliva start the neutralization process and then brush your teeth thoroughly.

Quit smoking: It goes without saying that smoking is bad for your lungs and can lead to many forms of mouth, throat and lung cancer. But did you know that tobacco also lodges in the crevices of your teeth and causes staining? Kicking the habit is an investment in many facets of your health.

Eat healthy: Not only is it helpful to reduce or eliminate certain foods, beverages and tobacco from your diet, but it’s also important to make sure you’re consuming a sufficient amount of vitamins and minerals. Calcium is found in several vegetables and dairy products and helps strengthen enamel. Vitamin D is also important because it helps your body absorb calcium. Phosphorus works with calcium to build strong bones and teeth. Potassium, vitamins C, K and A contribute to dental health as well. In addition, incorporating crunchy fruits and veggies into your diet can keep your teeth clean and staying hydrated with plenty of water each day keeps bacteria from building up in your mouth and causing tooth decay.

Practice daily dental care: The single best way to prevent tooth discoloration is by brushing and flossing faithfully each day. Brush in the morning and evening (more often if you eat sugary, acidic, or highly pigmented foods) and floss at least once a day. Additionally, you can incorporate a daily swish of fluoride mouthwash to cleanse your mouth from residual food particles and bacteria and keep enamel strong.

Cosmetic whitening solutions

In a perfect world, we would all make lifestyle choices that ward off tooth discoloration and enjoy white smiles without intervention. However, for many reasons—some that are out of our control—our teeth become dull and stained. Over-the-Counter vs. Professional Teeth Whitening

The relatively low cost of teeth whitening combined with its track record of proven results makes it a popular choice. In 2018, over 40 million Americans whitened their teeth – either at home or professionally. But are all teeth whitening methods created equal? Let’s explore the facets of both so you can decide which method is right for you!

Over-the-Counter Teeth Whitening

Strips: Because hydrogen-peroxide coated strips can be purchased for under $40, they are a popular go-to for people who are looking for whiter teeth. However, the strips do not provide excellent coverage for the crevices of teeth and the part of the teeth along the gum line and in between the teeth may be missed. Extreme tooth sensitivity is common and they only whiten teeth from two to four shades.

Toothpaste/Mouthwash/Gum: While these are certainly the most affordable option, they are best used in combination with other methods as they only whiten teeth by removing surface stains and typically the results aren’t very noticeable.

Tooth Discoloration: Causes and Prevention

Tooth discoloration can be an embarrassing situation that often leaves people feeling self-conscious and trying to stifle their smiles. Discoloration can be categorized as extrinsic or affecting only the surface of the tooth, intrinsic or affecting the inside of the tooth or age-related – discoloration that occurs as enamel wears away over time. Tooth discoloration can appear yellow, brown, grey, white, black or purple, depending on the cause. Let’s take a look at some culprits of tooth discoloration, how to remedy it and how it can be prevented.

Causes of tooth discoloration

There are several causes of tooth discoloration. A few of them include:

Foods: Highly pigmented foods such as tomato-based sauces, chocolate, berries and beets can cause yellow or brown discoloration.

Beverages: Beverages that contain tannins, such as coffee, tea and red wine can cause a yellow, brown or purple tinge.

Tobacco products: Cigarettes, cigars and chewing tobacco can result in brown or dark yellow teeth.

Aging: Tooth enamel wears away over time, exposing the inner layer of the tooth (called dentin) and leaves behind a yellow appearance.

Antibiotics: Tetracycline stains the enamel of unerupted teeth, resulting in a banded grey or brown appearance.

Injury: Blood flow can be cut off to a tooth, causing it to turn grey.

Teeth whitening

There are a couple of options when it comes to teeth-whitening.

Over-the-counter whitening: Options include whitening strips and toothpaste/mouthwash. These whitening options are thought to be affordable, however, results are subtle as teeth only whiten an average of 2 shades. Over-the-counter whitening strips can also result in significant tooth sensitivity. These methods are best used in combination with another professional whitening method.

Professional whitening: At Eggert Family Dentistry, we offer four whitening optinos – KöR Whitening Deep Bleaching, Zoom Whitening, at-home trays with Opalescence bleach, and Opalescence Go!:

Prevention

While not all teeth stains can be avoided, bruising and flossing after eating foods notorious for staining teeth can keep the pigment from attaching itself to the tooth. Also, according to BMC Public Health, “Twenty-eight percent of smokers reported having moderate and severe levels of tooth discoloration compared to 15% in non-smokers.” Clearly, limiting or eliminating the use of tobacco products is another powerful step in warding off tooth discoloration.

A New Smile with Veneers – Melanie’s Story

How did this start?

Melanie is a long-time patient of the practice. In 2014, she talked with Dr. Elizabeth about improving the appearance of her smile. Two of her upper front teeth were shorter than she liked and she wanted to know her options for lengthening them. Dr. Elizabeth recommended doing either composite or porcelain veneers and discussed the two options with Melanie. At that time, Melanie decided to go with composite veneers, which are fabricated chairside and only require one appointment to complete. Before doing the composite veneers, Dr. Elizabeth created a wax mock-up of Melanie’s new teeth to show her what they would look like. After the composite veneers were done, Melanie was very happy with her smile and the look of her new teeth.

Fast forward 4 years…

Melanie is now engaged and wants to brighten her smile for her upcoming wedding. Unfortunately, composite veneers will stain over time and Melanie wanted to do more bleaching to brighten her whole smile. She wanted to know what her options were for professional whitening, as she knew that at-home bleaching wouldn’t work on her existing composite veneers. Dr. Elizabeth explained to Melanie that even professional treatments wouldn’t whiten her veneers, but there were other treatment options she could consider to achieve her desired results.

What was involved?

Dr. Elizabeth developed a treatment plan for Melanie that consisted of three phases. The first was to get Melanie’s teeth into the proper position in preparation for the restorative work that would be done in the final phase. Melanie had started developing some spacing between her upper front teeth, so Dr. Elizabeth made her a retainer that was designed specifically to close this spacing. Melanie was very compliant in wearing her retainer as instructed and after four weeks, the space was closed and she was ready for the next phase of her treatment. To get Melanie’s natural teeth to her desired shade, Dr. Elizabeth recommended Zoom whitening, our in-office whitening treatment. This procedure is completed in a single session and provides fast, dramatic results. Melanie was very happy to see that the Zoom whitening system brightened her natural teeth by five full shades. Dr. Elizabeth then completed Melanie’s treatment by replacing her existing composite veneers with porcelain veneers that matched the shade of her newly whitened teeth.

What does she think?

Melanie said she was surprised by how quickly her treatment was able to be completed, as she had initially expected it would be a long process. She said she also felt well-informed and involved in her treatment plan.
